چکیده

Introduction Microincision of the cystic duct and its confluence part, which facilitated application transcystic-laparoscopic common bile (CBD) exploration (transcystic-LCBDE), had been described in some reports. However, it is unclear whether transcystic-LCBDE with microincision (micro-LCBDE) effective safe as transductal-LCBDE for failed patients choledocholithiasis. Methods In this retrospective cohort study, we assigned cholecystocholedocholithiasis to undergo laparoscopic cholecystectomy. The clinical outcomes one-to-one propensity-matched pairs Micro-LCBDE groups were compared. Results Between January 2007 December 2018, 1650 confirmed choledocholithiasis attempted transcystic-LCBDE. was done successfully 128 (7.8%) 69 (4.2%). By propensity score matching, 68 selected. There no significant difference in-hospital complication rate: 8.8% micro-LCBDE group 11.8% (p = 0.573). duration operation a little shorter than that transductal-LCBDE: median 125 (interquartile range 95-173) versus 150 (120-195) min respectively 0.003). Postoperative recovery faster transductal-LCBDE, reflected by postoperative hospital stay (median 3 5 days, p 0.002, respectively). also rates retained stone recurrence CBD between groups. Conclusions Our findings suggest surgery transcystic-LCBDE, suggesting an acceptable alternative

Laparoscopic Transcystic Common Bile Duct Exploration: Advantages over Laparoscopic Choledochotomy

PURPOSE The ideal treatment for choledocholithiasis should be simple, readily available, reliable, minimally invasive and cost-effective for patients. We performed this study to compare the benefits and drawbacks of different laparoscopic approaches (transcystic and choledochotomy) for removal of common bile duct stones. METHODS A systematic search was implemented for relevant literature usin...

Transcystic Approach to Laparoscopic Common Bile Duct Exploration

BACKGROUND AND OBJECTIVES One-stage laparoscopic management for common bile duct stones in patients with gallbladder stones has gained wide acceptance. We developed a novel technique using a transcystic approach for common bile duct exploration as an alternative to the existing procedures. METHODS From April 2010 to June 2012, 9 consecutive patients diagnosed with cholelithiasis and common bi...

Laparoscopic exploration can salvage failed endoscopic bile duct stone extraction.

INTRODUCTION Conventionally, patients who failed endoscopic removal of common bile duct stones (CBDS) by endoscopic retrograde cholangiopancreaticography (ERCP) would be treated with open cholecystectomy and common bile duct exploration. Laparoscopic common bile duct exploration (LCBDE) is an established option for treating CBDS. The aim of this paper was to look at the feasibility of LCBDE as ...

Transient increase in the common bile duct diameter after laparoscopic and open cholecystectomy

Introduction: Increase in the common bile duct diameter after cholecystectomy has been reported in animal model. However, similar studies in the human resulted in controversy. Methods: Between 2008 and 2012, fifty two patients with clinical as well as radiological signs and symptoms of gallbladder disease were enrolled in this study. The patients underwent laparoscopic and open cholecystectomy ...

Preliminary experience with laparoscopic common bile duct exploration

BACKGROUND Herein we present our experience with laparoscopic common bile duct exploration (LCBDE) in managing common bile duct stones. METHODS Data of 129 consecutive patients who underwent laparoscopic cholecystectomy (LC) and LCBDE done at our institutes from April 2011 through June 2016 were prospectively recorded and retrospectively reviewed. RESULTS Since 2011, 3012 laparoscopic chole...
